How to use your caregiver checklist
- Download and print: Print the PDF on standard US Letter paper. The worksheet is designed to be filled in by hand.
- Prepare together: Ask what the person wants to discuss and how they would like you to help. Write down their priorities and gather relevant information.
- Support the conversation: Let the person speak for themselves as much as they wish and are able. Share specific observations and take notes with their agreement.
- Agree on next steps: Confirm the plan with the healthcare team. Record who will handle each task and by when, then share notes with the person's permission.
Ask the provider's office about any permission or documentation needed for your involvement. Confirm unclear instructions with the healthcare team.
